The annual membership investment is $1,000 per patient, per year (non-refundable), which
allows me to continue providing the level of attentive, relationship-based care each family
deserves.

How do I enroll ?

Simply call our office at 847-441-1000, and our New Patient Coordinator will warmly guide

you through the enrollment process.

How does the membership term work?

Your one-year concierge membership begins on the day your membership agreement is

signed and includes 365 days of care and access to our practice.

Do you offer a payment plan for the membership fee?
At this time, the annual membership fee is required in full and is non-refundable.

Will my child always see Dr. Franzia?

We make every effort to accommodate requests for Dr. Franzia; however, we cannot guarantee
availability for every visit. Rest assured, your child will always receive attentive, personalized

care from our trusted team.

Do I need to be a member for ear piercing services?
No, ear piercing services are available to both members and non-members.
